The most common side effect is redness of the skin, especially when you first start using the product. In fact, it's very common that your acne may appear worse because of the redness. All of these formulations come in a variety of different vehicles including creams, gels, and solutions.
How To Apply
Start with the lowest strength to avoid possible irritation. Typically it is recommended to start dosing at three times weekly, progressing to nightly use as tolerated. All retinoids are applied in the evening before going to bed. Ideally, you should wash the face with a non-medicated soap or gentle cleanser (such as Cetaphil), then apply a thin film of the cream, lotion, or gel to cover the entire face. Do not rinse off afterwards.
Important Additional Information
All of the retinoid products work in the same way, but certain products and formulations are either better tolerated or more effective. If what your doctor prescribed you isn't having the desired effect, it usually helps to change the retinoid product, strength or formulation.
Tretinoin creams are the most commonly prescribed since they are available generically and are less expensive than other products. They do tend to cause the most irritation in the retinoid class however. There is a tretinoin product known as Retin-A micro which is the best tolerated of the retinoid bunch. Tretinoin is deactivated by sunlight and therefore should be applied at bedtime.
Adapalene (Differin) products are considered to be slightly more efficacious than tretinoin products and certainly cause less skin irritation. Some formulations of adapalene are available generically just as tretinoin products are but in general does cost more. Sunlight deactivates adapalene just like tretinoin and therefore should be applied at bedtime.
Tazarotene (Tazorac) is considered the most effective retinoid of all but is typically more irritating that the others. It commonly causes redness and scaling. Also, tazarotene is absolutely to be avoided in pregnancy or in those who wish to become pregnant.
For all the retinoid products, as the strength increases so does the possibility of redness, flaking, and irritation. Ideally you want to start at a couple of times per week and move up to nightly dosing.
In terms of picking a product formulation (cream vs gel etc.), the general recommendation is to pick a formulation based on your skin type. For people with more oily skin, it is recommended to go with a gel or solution because they contain more alcohol and tend to be a little bit more drying. In people with dry, flaky skin, cream is usually the best choice as it won't dry as much.

Azelaic Acid (Azelex) is usually considered slightly less effective than the retinoid products but again, it is great for people with sensitive skin. In those people with darker complexions, it has been know to cause hypopigmentation, also known as a loss in skin color.
Dapsone (Aczone)
When To Use/Place In Therapy
Dapsone (Aczone) is a topical antibiotic formulation indicated for mild, moderate or even severe acne. It usually is used in combination with other products. Though Dapsone is classified as an antibiotic, it appears to work by preventing and treating the inflammatory process. It is usually very well tolerated and typically is regarded as a good option for patients that can't tolerate other medications well.
How To Apply
Dapsone should be applied to a clean and dry face. Dapsone does tend to be a little bit gritty so you can expect to feel that when you rub it in. Do not rinse your face afterwards.
Important Additional Information
Two things are important to know about Dapsone. First, it is usually fairly well tolerated and should not make acne worse before it gets better, unlike the retinoid products. Secondly, it usually is not used with benzoyl peroxide products as the combination of both can cause a temporary orange staining on the skin.
Spironolactone (Aldactone)
When To Use/Place In Therapy
Spironolactone is an oral therapy and is usually reserved for more moderate to severe cases of acne where other treatments haven't worked. It is almost always used in combination with topical products and is almost exclusively used only in females with either excess androgenization (i.e. excess male hormones) or acne that comes and goes in cycles. Excessively oily skin, abnormally excessive hair growth and menstrual irregularities sometimes signal excess androgen in women.
How To Apply
The dosage for spironolactone is individualized in each patient with typical dosages ranging from 25 to 200 mg. Ideally you want the lowest effective dose. It should be taken in the morning because it is a mild diuretic (causes a loss of water through urination).
Important Additional Information
Spironolactone is know as an anti-androgen and is very effective in reducing sebum (oil). It works by blocking the conversion of testosterone to dihydrotestosterone. Excess Dihydrotestosterone can cause many dermatological issues such as clogged pores and inflammation. Studies have shown that spironolactone reduces sebum excretion in the skin by about 30-50 percent and therefore is very effective in those with oily skin.
Being an oral medication, it has the possibility to cause more side effects than topical treatments. The most common side effects are excess urination, menstrual irregularities (spotting etc.) and low blood pressure. The menstual irregularities are usually decreased in woman who are also taking oral hormonal contraceptives.
Many compounding pharmacies make topical spironolactone which can sometimes help reduce side effects. It is not commercially available though at retail pharmacies.

Topical antibiotics should never be the sole treatment for several reasons including the extended time it takes to see positive effects and the fact that bacterial resistance can occur. It's important to note that acne is not an infection. Everyone carries a bacteria in their skin known as P. acnes. The problem is when the bacteria over grows in the skin. Some byproducts of the metabolism of the P. Acnes bacteria are irritants to the skin and lead to microcomedo formations and results in inflammatory lesions. Decreasing the amount of P.Acnes via antibiotic therapy is usually associated with acne improvement.
Topical antibiotics include, erythromycin and clindamycin.
How To Apply
Topical antibiotics are applied usually once to twice daily. Apply to a clean, dry face and do not wash off after application
Important Additional Information
Topical antibiotics typically are used in 3 month spurts and then given a break. They are always to be used in combination therapy with other acne products such as benzoyl peroxide.

You may not care to know this, but the gastrointestinal tract of the average human contains about two to three pounds of bacteria, which outnumbers the body cells by about ten times. When we normally think of bacteria we imagine disease-causing microbes that invade our bodies. There are certainly plenty of those, but our gastrointestinal tract is made up of a complex system of microbes. They are not only bacteria, but fungi, yeast and protozoa that all carry out specific tasks for our health. They are found in both the large and small intestines.
Two of the strains of bacteria are firmicutes and bacteroidetes, and these two dominate the gut flora of humans as well as other vertebrates. The mix of these two differs in lean and obese people. It has been found that firmicutes bacteria are in a greater abundance in test animals that were obese, and the level of obesity was directly proportional to the levels of firmicutes. The actual cause seemed to be in the fatty acid absorption and the ability to turn calories from sugars into fats.
The bacteroidetes strain, on the other hand, was found to be in higher levels in lean people. It seems that if we are able to shift the balance in these two bacteria in our digestive system it would go a long way to promote weight loss. In tests even with alterations in the diet the balance between the two remained fairly constant over time. So does this suggest that the variation in bacterial composition can't be changed merely by changing our diet?
The answer seems to be that not in the short-term. In a small human study it was found that true to what we know individuals with fewer bacteroidetes (good) and more firmicutes (bad) had greater weight problems than their leaner counterparts. But when the obese participants went on a low-fat low-carbohydrate diet for better than a year, an increase in bacteroidetes in proportion to firmicutes was found.
So diet can turn us round, and if we keep on it in time we should see improvements in our health and make controlling our weight easier. What would some of these foods be?
1. Organic food. Pesticides and other chemicals can play havoc with your beneficial bacteria. Buy organic food products as much as possible, but some fruits and vegetables are especially prone to harmful pesticides if not organic. Not only chemicals that come on foods, but all pollutants that get into the system can do damage.
2. Fermented foods. Sauerkraut, fermented vegetables and even pickles help with gut bacteria.
3. Probiotic foods and supplements. Yogurt has become a favorite, but there are many products such as Kefir that fill the bell.

For example if you are a milk-drinker, switch to a lower fat version-1% or non-fat skim milk. These milk options will still contain calcium and vitamin D.
Breads, rice and potatoes are carbohydrates which provide the energy we need to fuel our bodies. We need carbs; however high-fiber carbohydrates are the best choices. Look for whole-grain breads and rolls, and choose brown rice over white rice. These are small changes to good carbohydrates that are higher in fiber. If potatoes are one of your favorites, add sweet potatoes to the mix. They can be baked whole, served mashed or prepared as oven fries. What good does it do to set goals for weight loss, better health, more energy, lower blood pressure, lower cholesterol - when you can't get those goals because your food addictions sabotage you?
Are you at risk for diabetes? For heart disease or hypertension? Is your cholesterol too high? Has your doctor told you to lose weight to reverse those conditions? Have you been unable to follow your doctor's instructions because you can't stay away from your "trigger" foods? Does it sometimes feel as if those forbidden foods are all you want to eat?
The foods you can't stop eating could be harming your health. You're not alone.
Food addictions are real. They're not psychological, emotional, or "mental." They're physiological and based on your brain chemistry.
We know our health matters. Probably everyone agrees that what we eat affects our health. Nutrition approaches to health vary widely. Some people focus on organic foods, some on raw foods, others on herbs, phytonutrients and colorful pigments. There are low-carb, macrobiotic, and Ayurvedic diets, and a lot more.
One key, often overlooked, health factor is prostaglandin production. Prostaglandins are short-lived, hormone-like chemicals that are produced within cells and move from cell to cell, rather than through the bloodstream. They regulate all kinds of cellular activities.
Inflammation has become a hot topic in medicine. More and more research indicates that most disease begins with inflammation. Inflammation is caused by a specific prostaglandin - and foods influence prostaglandin production.
There are three types of prostaglandins.
Series 1 control factors that we recognize as beneficial. They dilate blood vessels, reduce blood pressure, inhibit unnecessary clotting, decrease autoimmune disease risk, improve T-cell function, improve insulin sensitivity, decrease pain, decrease inflammation, decrease the need for sleep, alleviate depression, and much, much more. Space limits a longer list.
Series 1 prostaglandins are made from dietary fatty acids in the omega-6 category. These include black current seed, flaxseed, hemp seed, pumpkin seed, walnuts, borage oil, evening primrose oil, sesame oil, sunflower oil, and more.
Series 2 do basically the opposite of the Series 1 functions listed above (as well as the unlisted Series 1 functions). Series 2 prostaglandins promote pain and inflammation, making them a prime culprit in disease.
Series 2 prostaglandins are also made from omega-6 fatty acids. However, high insulin secretion causes a different enzyme to act on the omega-6 fats, resulting in production of Series 2, rather than Series 1. As covered in previous posts, high insulin triggers include sugars, processed carbs (like white bread), saturated fats, alcohol, and others.
Series 3 prostaglandins reduce the negative effects of Series 2. They decrease inflammation and enhance immune function. Series 3 are made from omega-3 fatty acids, which we hear about often these days because they have anti-inflammatory benefits. So they can reduce what's presently considered the cause of most disease.
Omega-3 fatty acids include dark green leafy vegetables, fish, seeds (black currant, flax, hemp, and pumpkin) and walnuts.
To bottom-line this and get the health benefits:
1. Consume omega-3 fats. You'll note that many of them also contain omega-6s, so you'll get the precursors of both Series 1 and Series 3 prostaglandins.
2. Avoid junky carbs and limit alcohol. Both trigger lots of insulin, which leads to Series 2 production.
3. Avoid all-carb meals or snacks whenever possible. Eating protein and healthful fats EACH time you eat can reduce the insulin impact of your diet and Series 2 production. This could decrease the incidence of both serious diseases and inconvenient ones, such as colds.

Foods that supply all of the essential amino acids, like meat, milk, eggs and cheese, used to be called 'complete' proteins, while those that do not were called 'incomplete' proteins. Complementary proteins referred to two incomplete proteins that together provided all of the essential amino acids.
These terms are not widely used to describe foods anymore, however, and the old adage that you had to eat complementary proteins at the same meal to count as a complete protein source has been disproven. As long as you consume all of the essential amino acids from a variety of protein-rich foods in the same day, you'll be fine.
Most Americans Eat More Protein Than They Need
Nutrient deficiencies are common in the United States, but protein is not typically among them. According to the U.S. Centers for Disease Control and Prevention (CDC):
"... most adults in the United States get more than enough protein to meet their needs. It's rare for someone who is healthy and eating a varied diet to not get enough protein."
Generally, you only need about 10-35 percent of your daily calories to come from protein. For an adult woman, this amounts to about 46 grams of protein a day, or 56 grams for men. For most people, two to three servings of healthful protein sources are enough to fulfill your daily protein requirements. Examples of protein serving sizes include:
  • 1/2 cup of beans
  • 3 ounces of meat or fish
  • 1 egg
  • 2 tablespoons of peanut butter
  • 1 ounce of cheese
What Happens if You Eat Too Much Protein?
Since your body can only use a certain amount of protein each day, if you regularly consume more than your body needs, the extra protein calories will be stored as fat, which can lead to weight gain (each gram of protein has four calories).
